Service, Treatment or Surgery is a required selection in the General Plan Information sheet. These refer to various categories of services, treatments or surgeries you might be hospitalized for. Select an item to use for your desired scenario in producing the Out of Pocket Estimate Report. Not every possible hospital inpatient service, treatment or surgery is provided in the list. Instead a number of representative and common items are provided in the listing, to give you a range of scenarios to choose from. If you are contemplating a specific type of hospitalization that is not provided for on the list, you might look for a selection that has some similarities to your situation. As an alternative to selecting a Service, Treatment or Surgery from the drop down list, you can enter an amount in the box designated "Or Enter a Total Cost:".
