Select a plan model

Select a Plan Model is a required entry in the Plan Information section of the General Plan Information Sheet. The models to select from include:

PPO High Benefit
PPO Low Benefit
HMO High Benefit
HMO Low Benefit
HMO Tiered Benefit
Point of Service
Consumer Driven Plan

Each model provides default entries for applicable items in each Sheet. After selecting a model, you can choose to either complete your other required entries in the General Plan Information Sheet and Calculate Your Costs based on the default information for that model; or you can choose to further customize the information based on a specific Plan you are using with your scenario, by entering your own applicable information in each Sheet and then Calculate Your Costs. If you are entering your own custom information for a Plan, you still want to start with a model that most closely approximates your scenario.
