General Plan Information Sheet

The General Plan Information Sheet is your starting point from the Main Menu, and includes required entries you must make to produce your Out of Pocket Estimate Report. You will need to provide the requested information to set the parameters for your scenario including your Demographics, Plan Information, the Service, Treatment or Surgery you have selected and your Coverage Information. After entering all applicable information, click the Update button, and also check the Calculate Your Costs box if you are through entering all applicable information and want to view the Out of Pocket Estimate Report.
