Demographics

The Demographics section of the General Plan Information sheet has two required entries: Age Group (select one) and Sex (Male or Female). This information is providing the estimated cost and utilization of the Service, Treatment or Surgery you have selected, so that applicable out of pocket calculations can be determined from these estimated amounts.
