Deductibles/Maximums Sheet

The Deductibles/Maximums Sheet allows you to specify any General Deductible, Hospital Deductible (or copay per admission) and any Maximum Annual Out of Pocket limits. You would gather this information from your health plan’s Summary of Benefits or similar document available in their web site or plan materials. You do not need to enter any information if you simply wish to use the default information provided with the Plan Model you selected in the General Plan Information sheet. After entering any information, click the Update button, and also check the Calculate Your Costs box if you are through entering all applicable information and want to view the Out of Pocket Estimate Report.
